The Power of the Care Team and Beyond
- Travel with us as we create a path to success using innovative technologies for community-based settings of care and services. We will demonstrate a roadmap strategy for technology-supported care delivery that leverages an untethered consumer Personal Health Record (PHR) app, care team communication tools, and technology features that improve care quality, improve care safety, and enable better care experiences for patients and their natural supports.
Technology Applied to a Solution: Mental/Behavioral Health Use Case in Senior Living
- Knowing that something works is important before we try that something. Having examples is key to taking success and spreading it to other organizations. Understanding that the same outcomes are possible in my organization is critical. In this session we explore how a provider organization and those they serve are benefitting from technology solutions that improve operational efficiency, lower overall costs, and increase the quality of care for individuals needing mental/behavioral healthcare in the senior living setting. Session provided by facilitated panel discussion.
Participant Polling: “What keeps you up at night?”
- Community-based care settings are critical to the overall population health in any community. Decision-makers in this space understand the need for operational efficiency and a strong business model to succeed. This session provides an opportunity for the voice of the consumer to be heard. The discussion will use ‘live polling’ to address the process, people, and technology issues encountered in today’s care systems. Topic areas for inclusion are:
- Care team management
- Occupancy challenges and managing the business
- Staffing for quality outcomes
- Regulatory pressure
- Customer acuity challenges
